Myths about Testosterone Therapy You Need to Know

Testosterone Replacement Therapy (TRT) is an effective medical treatment for men with diagnosed hormonal deficiency. However, it has been the subject of many myths and misunderstandings, largely due to confusion with anabolic steroid abuse. It is essential to differentiate medical, safe, and supervised TRT from the misuse of steroids for cosmetic or athletic purposes.

Myths and Realities About Testosterone and TRT

Myth 1: TRT causes prostate cancer.

Truth: This is one of the oldest and most widespread myths. It is based on a decades-old belief that testosterone “feeds” prostate cancer. However, more recent studies have shown that there is no direct causal relationship between TRT and the development of prostate cancer in healthy men. The key is medical supervision: before starting treatment, a specialist must rule out any signs of active prostate cancer. Regular monitoring, including prostate-specific antigen (PSA) testing, is mandatory.

Myth 2: Testosterone causes uncontrolled aggression (“roid rage”).

Truth: This myth is a direct extrapolation from anabolic steroid abuse, which is used in massive doses. Medical TRT uses physiological doses to restore hormone levels to a normal range, not to surpass them. While the therapy may cause increased irritability or mood swings in some individuals, it does not produce unmanageable aggression. Using the correct dosage and medical supervision are crucial to mitigating this risk.

Myth 3: TRT is an anti-aging “magic pill.”.

Truth: Testosterone is neither a fountain of youth nor a universal cure-all. Its use is indicated for men with clinically diagnosed hypogonadism (testosterone deficiency). Administering it to men with normal hormone levels does not produce the same effects and can be harmful. While therapy can improve vitality and mood, it is not a remedy for aging in general. It is a serious treatment for a specific medical condition.

Myth 4: Over-the-counter supplements are effective.

Fact: Most over-the-counter testosterone pills and supplements have not been shown to be effective in raising hormone levels. Prescription testosterone must be administered by injection, gel, or patch to ensure proper absorption. Self-medicating with these products, or with illegally obtained testosterone, is extremely dangerous, as the dosages are inadequate and there is no medical supervision to monitor side effects.

Myth 5: TRT causes permanent infertility.

Truth: TRT can reduce the natural production of testosterone and sperm, affecting fertility. This effect is reversible in most cases upon discontinuing treatment, but it can be an important factor for men planning to have children in the short term. However, specialists can manage this risk with parallel strategies, such as the administration of gonadotropins, to preserve fertility.

Myth 6: TRT always causes acne and hair loss.

Truth: These are possible side effects, but not universal. Their appearance is linked to the dose and the individual sensitivity of the patient. A specialist can adjust the dose or change the route of administration (e.g., from gel to injection) to mitigate these effects. Male-pattern baldness is a side effect that can be accelerated by TRT in men with a genetic predisposition.
